Leisha Hubbell, Elder Care Advocate

Leisha Hubbell, RN works as an Elder Care Advocate in the firm’s Elder Law and Benefits Team and is responsible for planning, directing, and coordinating medical and health services for our clients. Leisha has extensive home care experience and knowledge of the community-based services and has worked as a Hospice nurse for Washington County Public Health and as a nurse evaluator for the Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) waiver programs. Prior to joining the firm, Leisha worked as a RN supervisor and for the last two years had served as a care manger for a Managed Long Term Care as well as providing Service Coordination for the TBI and NHTD waiver program for a major local home care provider.

As a member of our team, Leisha conducts care assessments to coordinate health care services that will best meet the client’s and family’s long-term care goals. She assists with preparing and guiding the client and family through the Medicaid application process and the difficulties of structuring care for clients or their loved ones. Leisha is based primarily from the firm’s office located in Queensbury, New York, is available at the firm’s main office located in Clifton Park, New York, but is most often meeting with clients at their homes. Leisha is a graduate of Adirondack Community College in Queensbury NY with an AAS degree in nursing magna cum laude.
